Instructions for Electronic Submissions
The Committee on Academic Misconduct (COAM) requests that you submit the following materials when reporting allegations of academic misconduct:
- A letter from the Department Chairperson (or other administrative officer if appropriate) acknowledging that he/she is aware of the allegations of academic misconduct;
- A completed "reporting template;"
- Copies of all evidence supporting the allegation (if you are reporting allegations of plagiarism, see here for suggestions on how to prepare the evidence), including any supporting documents (e.g., e-mails, print outs of web pages or file properties, photocopies of exams, etc.); and
- A copy of your course syllabus.
A hard copy of the above documents may be submitted to COAM (33 W. 11th Avenue, Rm. 107) by campus mail, or the information may be submitted electronically using either of the following protocols:
- Prepare and accumulate all of the above documents in the order listed. Scan all of the documents and save as a single .pdf file. Send the file via your OSU e-mail account to the Coordinator of COAM (curry.6@osu.edu); or
- If it is more convenient, you can send the above documents as separate files in different formats as e-mail attachments. For example, the Chairperson's letter can be sent directly to the Coordinator as an e-mail (or scanned and sent as a .pdf file), the "reporting template" can be sent as a Word document (.doc), and the evidence can be submitted in whichever format is appropriate (e.g., .xls, .pdf, or .doc files, etc.). If you submit the "reporting template" electronically as a separate file, no signature is required.
