Reporting Template and Instructions


The Committee on Academic Misconduct (COAM) has prepared a template for submitting allegations of academic misconduct. We hope that you will use this template, or at least follow its format, when submitting an allegation of academic misconduct, since doing so will help to insure that COAM receives all of the information needed to resolve the case. If you have any questions about using the template, please call COAM's office.
