Six Easy Steps for Submitting Allegations of Academic Misconduct



  1. If you suspect that a student has violated the University's Code of Student Conduct, we (COAM) recommend that you notify the student of your suspicions. (This notification is not mandatory!) This can be done via a personal, private conversation (a witness is recommended) or via e-mail.

  2. Download and complete a copy of COAM's reporting template.

  3. If you suspect that a student has plagiarized an assignment, read Preparation of Evidence for Cases Involving Alleged Plagiarism.

  4. Ask you department's chairperson (or other administrator as appropriate) to write a letter acknowledging that he/she is aware of the allegations against the student.

  5. Accumulate the following documents:

    CAMPUS

    Or, submit the above documents electronically.
